1. In a blender, combine the milk, heavy cream, sugar, and vanilla extract. Blend until smooth. 2. Pour the mixture into a shallow dish and freeze for at least 4 hours, or until firm. 3. When the mixture is frozen, remove it from the freezer and break it into small pieces. 4. Place the pieces of frozen mixture into a blender and add the crushed ice. Blend until smooth. 5. Pour the slushy into a glass and top with whipped cream and garnishes of your choice. 6. Enjoy!
